Organization Information FORM

This form must be completed by every Student Organization. It contains all the information required by the LALSS for student organizations to have access to LALSS online resources including the Organization Directory and Forms in the Organization Hub.

This form must be completed by the leadership of your organization.

Directory Information

Please fill out this form to the best of your ability and include a square logo. Organization description, contacts and logo will be included in the Organization Directory on the LALSS main website.

Key Contacts

Key contacts are important parts of approval and review processes including reimbursement requests and event and calendar submissions. Please ensure that this information is correct.
